My Own Place. Inc. s committed to designing and providing quality supports and services to meet the individual needs of each person. Through a wholistic approach to quality of life, the primary focus is to assist people to live their lives as independently as possible based on their needs and dreams.
Alongside our funding from Medicaid Home and Community Based Waiver for people with intellectual and developmental disabilities, Agency mission and policies incorporate the core concepts of quality of life, dignity of risk and empowerment to ensure that, My Own Place Inc., is meeting the needs of all supported.
My Own Place, Inc. is committed to providing quality supports to people with disabilities in order to help them live their lives as independently as possible. We believe that all people should be treated with dignity and respect, and that everyone should have the opportunity to reach their full potential.
Supports and Services
Supported Living with Transportation
This model allows individuals to live alone or with one or two housemates, typically in an apartment setting, with up to 24-hour staff support for tasks they cannot complete independently. Support levels are decided by the individual and their support circle (family, DDS, support coordinator, advocate, attorney) during regular interdisciplinary team meetings and based on clinical evaluations. For those eligible under the Home and Community-Based Waiver, support varies, with many receiving 20-30 hours per week due to demonstrated independence in basic skills.
"My Own Place" provides transportation through a fleet and ensures all staff are qualified drivers. Clients capable of using public transit receive training and assistance, and eligible individuals are granted Metro Access.
Service delivery is guided by each person’s annually developed Individual Support Plan (ISP) and Prior Authorizations, which specify approved services. Additional funding from DC Human Care Agreements covers expenses like rent, utilities, communication, and clothing not covered by the waiver.
Supported Living Periodic
This service is similar to Supported Living with Transportation but is designed for individuals with fewer support needs, approved only for the specific level of assistance required. For instance, someone may receive 20 hours of weekly support for key tasks. Staff schedules align with these needs as outlined in the Individual Support Plan (ISP) and approved authorizations. If support needs change, adjustments in prior authorizations are made to secure the necessary funding.
Host Home
My Own Place offers support through private homeowners or renters who host one person in their home. Supervised by a team (QDDP, RN, and Program Manager), the Host Home model resembles adult foster care, with the host providing living space, home access, and assistance with daily activities as needed. The RN monitors healthcare, the Program Manager oversees the home environment and personal items, and the QDDP ensures the Individual Support Plan (ISP) is implemented.
Residential Habilitation 24 hr supports
This program is intended for individuals needing more assistance and skill development than those in Supported Living, providing 24-hour daily staff support. Participants often have limited community and home survival skills and require additional clinical supports (e.g., PT, OT, behavior support, nutrition). Support needs are outlined in each person’s Individual Support Plan (ISP) and Prior Authorizations, informed by clinical assessments.
The Host Home model offers care through private homeowners or renters supporting one person in their home. My Own Place supervises these homes with a team (QDDP, RN, Program Manager), who conduct visits and audits. Similar to adult foster care, the host provides living space, access to the home, and assistance with daily living activities, while the team monitors health care, personal items, finances, and ISP implementation.
