Promoting Health and Safety of each Person
In addition to providing homes to our participants, another key component of My Own Place is promoting the health and safety of each person. Each participant receives quality, individualized preventative and acute health care in an accessible community setting with a primary care physician who is knowledgeable about the needs of older adults. Agency policies and procedures have been developed for dealing with medical emergencies and acute illnesses as they arise.My Own Place provides transportation services for medical appointments, along with accompaniment of a staff person who best knows the program participant, when needed.Funding for health care services is through Medicaid, Medicare, or by private insurance for individuals who are gainfully employed. Routine physical examinations and laboratory tests are completed annually. In addition, acute care illnesses are treated and follow-up visits are done by physician recommendations. Nurses and administrative staff meet at least monthly to discuss the health needs of each individual and to ensure that follow-up services are being delivered in a timely manner.
